Your Pathway to Personalized, Integrative Care
Our “Harmonize Your Health” program begins with a 90-minute Comprehensive New Patient Evaluation, where we take a deep dive into your health history, current symptoms, and wellness goals. This initial consultation includes:
In-Depth Review: We analyze your health questionnaire, past labs, medical history, and any prior records you upload to our secure Charm EHR system.
Active Listening: Dr. Lewis listens to your story, paying close attention to overlooked details.
Physical Examination: For local patients, we conduct a physical exam to gather further insights. Remote patients are asked to submit their most recent physical results from their primary care physician.
Personalized Lab Testing: We recommend necessary lab work and may provide take-home test kits, draw blood during the visit, or schedule a follow-up appointment.
Immediate Recommendations: You’ll receive 1-2 initial health recommendations to begin implementing right away.
Following this initial evaluation, we’ll introduce you to the Harmonize Your Health membership program, outlining the benefits, services, and savings available to you.
